Wrap Up

You must complete all of the Steps listed below by June 15th, 2007 in order to qualify for winning the Prizes.

Step 1. Finish Recording Activity

After you have finished recording all of your activities in your Get Fit Journal, locate the link for the Application Form.

Download the Application Form by clicking on that link.

Print the Application Form.

Call the same Clinic and make an appointment to visit them for another Fitness Assessment.

Take the Application Form with you to the Clinic.

The Clinic will:

Give you another fitness assessment.

Fill in the bottom portion of the Application Form that is titled: MEDICAL INFORMATION

Send in the Application Form for you.

Look for the messages that say: Here are the Prize Winners and here is what they won...???.
